When I was a medical clinic administrator in the early 1970s, the funeral  
home filled out most of the vital information on the deceased, then sent it over 
 to the physician in attendance at the time the patient expired. The cause of 
 death and his/her signature was put on the death certificate, then forwarded 
 back to the funeral home. For OK. and AR., the funeral home was who sent in 
the  death certificates to the state.
We did not keep a copy at clinic of the death certificate.
