Subject: [ROOTS-L] Blank World War I registration card?

Looking for the text of the questions on a World War I
registration card.  On the one I'm looking at, the man's
answers are large and fairly readable, but the questions
on the (scanned) card are small and illegible.

In case there are different kinds, this one is 2 pages
(sides?).  The first one says REGISTRATION CARD at the
top.  The second one says REGISTRAR'S REPORT.
