Russell,
Submit the request for the death certificate and put as much information on  
the request as you have available. 
 
You wouldn't be the first one that didn't fill out all the information that  
they asked for (guess who? - grin). 
 
Sometimes you win and sometimes you lose but the worst they can say is they  
can't find it. I have filled out some with minimal information and received 
the  death certificate and other times I have filled in everything asked for and 
 received a sorry.
 
The worst, and best case, was my Aunt Mary Flynn (Such an uncommon name -  
grin).
 
I had the month of the date of death in Chicago, Cook County, Illinois plus  
the date born. Did not have her parents names, and only remarked that it was  
possible she was born in Minnesota. Received a call from a very nice lady in 
the  Cook county records department. 
 
Seems they had multiple Mary Flynn's that sort of fit the one I was looking  
for. I thanked her for her time and told her the only other thing I could 
think  of was her husband's name. She said, oh here it is I'll have it in the mail 
for  you tomorrow. Luckily I always put my phone number and email on 
everything when  I ask for something.

I thought if I bought a death certificate of  my Grandmother's that her 
parents
might be listed but in filling out the  request, one of the questions was who
her parents were.  The only thing  I have found pertaining to my Grandmother 
is
a picture of her headstone which  only shows her lifespan and who she married.
Does anyone have any  suggestions?  My father remembers visiting his
Grandparents but does not  remember their names.  I have nearly exhausted the
family history  route.
