Subject: [ROOTS-L] Re: Social Security Application, Need Answers

Hello Melissa,
The only place you can order a copy of a deceased person's SS-5 form
is from the Social Security Administration. I don't think there's
another way to get them. You could order a copy of the numident
(computer print-out) for a little less money, but numidents don't have
the same information on them.

SS-5 Form = Application for a Social Security Card/Account Number
(SSN). For an example (of a blank one) see...

http://www.researchguides.net/ss-5.htm

Numident = Computer extract of Social Security Number Application. To
see some differences between a numident and SS-5 see...

http://www.kidsgenealogy.com/askr021402.html
(scroll down to "Numident vs. SS-5")

Also see...
http://www.ssa.gov/foia/html/foia_guide.htm#newfees

The last time I ordered an SS-5 from the SSA it took about 6 weeks. It
might take longer depending on the number of current requests.

Good luck with your search.

Regards,
Joe
