Could someone please look up the following individual in the death certificate records for Lucas County, Ohio?

I am especially interested in knowing whether this certificate gives the names of his parents.  He _may_ be great uncle of mine.  I know that one with his surname lived in or around Toledo, but I do not recall his given name.
