Hi Richard,

I would think the family would decide.   The death certificate will show the
hospital because a witness (of the hospital)  is required to sign the
document.    The obituary will be prepared by the family.   If I die in a
hospital away from my home, I would want my family to identify the hospital
and also identify my home as where I lived a very happy life with my
family.   And, I would want my family to identify the "non-profit"
organizations I belong to that benefited my fellowmen and include the town
locations for those organizations.
