Subject: [ROOTS-L] California Death Certificate fields?

Recently I ordered & received a death certificate from California for a 
cousin.  While the entries were all clear, I can't read most of the field numbers 
and explanations.
This death certificate is from 1981.
Does anyone know what the various fields are or can you point me to someplace 
where I can find out?

I am particularly interested in fields 2B, 16, 22, 23, 24, 25, 26, 31, 32A, 
32B, 35A, 36, 37, 39, & 42.
