I've seen quite a few numidents and I've never seen the parents' names on 
one.  The SS-5 info should include the parents' names.

I think it merely means the file was updated for some reason--perhaps to 
correct that he died on an unknown day rather on the 1st of the month.

This would merely be the District Office that handled the claim--San Antonio 
(downtown office). Probably where the report of death was submitted.
