To obtain military records from the National Archives, write to:

and ask for copies of Form 80.  or   NATF Form 26                               
                                                                                
When you fill out the form, you should add a note with any additional           
information that might be helpful in the search.                                
                                                                                
When you return a completed Form 80, they will search certain records for       
that individual.  They will contact you to tell you what they found and         
what you have to pay to obtain copies.  The price is something like $5.00       
for the successful search plus $0.35 for each item.
